Why Community Health Systems Must Start on Site
Lessons from the field, and why so many good systems go unused.

Systems designed in meeting rooms tend to miss the one detail that matters most: how the work actually happens for the person doing it.
We regularly find staff entering the same information two or three times, because the new system was never connected to the paper record they still have to keep.
Going to site before designing is not a formality. It decides whether the system gets used or abandoned.
